History: Pelvic discomfort. Fever
What are the findings? Bladder with nondependent air and with dense heterogeneous material layering within the bladder, similar to contrast. Additional thickened colon with linear mesenteric stranding contiguous with the bladder.
What is the most likely diagnosis? Colovesical fistula related to colonic inflammatory process.
What should the radiologist do next? Surgical consultation.

 

Case of the Week From 9/17/23

History: Elderly male with altered mental status
What is the name of the sequence? Gradient echo
What is the differential diagnosis? amyloid angiopathy or hypertensive hemorrhages/angiopathy
Which etiology is most likely and why? Given the location of the susceptibility artifact centered at the basal ganglia, hypertensive angiopathy is more likely.

History: Lung lesion on outside study.
What is the name of the scan? F18 FDG-PMSA (Pylarify)
What is the differential diagnosis for the thoracic findings and why? PMSA avid left lower lobe mass with microlobulated borders can be primary lung neoplasm (some are PMSA avid) or atypical prostate metastasis. PMSA avid mediastinal nodes can be metastatic lung cancer or prostate cancer related. Renal cell carcinomas can also be PMSA avid.
