History: Surgery with lysis of adhesions two days ago.
What has changed between the two scans? Interval development of increased free air, increasing post-surgically that should instead be decreasing. New hematocrit level in the pelvis. Consider possibility of postoperative perforation, hemorrhage.
How would you manage the case? Surgical consult recommended for possibility of surgical intervention.

History: Pelvic swelling.
What are the findings? Large pelvic mass extending from the uterus with large cystic density component. Some fat contained within the mass closer to versus within the uterus.
What is the most likely differential diagnosis? Large pelvic teratoma or less likely leiomyolipoma/leiomyoliposarcoma

History: Chest pain. Shortness of breath.
What are the findings? Diffusely increased heart size
What is the most likely differential diagnosis? Pericardial effusion or dilated cardiomyopathy.
What is the recommended management? Correlate with echocardiography

Case of the Week from 11/7/2021
History: Headache
What are the findings? Rim calcified mass in the suprasellar cistern.
What is the most likely diagnosis? Aneurysm near the left MCA origin
Does the PET-FDG scan add anything to the case? Yes. Because the mass is not FDG avid, it is unlikely to be the next most common lesion in the differential diagnosis- a pituitary adenoma.
