History: Elbow pain.
Give the findings and diagnosis: Increased fluid surrounding the UCL with thinning of the UCL consistent with a partial tear.
How is this entity treated? Usually treated conservatively if it is a partial tear. Complete tears can be treated with a repair (Tommy John Surgery)


Case of the Week from 5/23/21

History: Pulmonary nodule. First CT scan from 1 year ago. Additional studies from today.
Give the findings: Enlarging right middle lobe ill defined peripheral pulmonary nodule with moderate hypermetabolism greater than mediastinal blood pool and SUVmax of 3.9.
What is the most likely diagnosis? Consider bronchoalveolar/low grade primary malignant lung neoplasm given configuration and moderate hyper metabolism. Other inflammatory/infectious etiologies are less likely given slow growth but are also within the differential diagnosis.

What is the most relevant finding? Disruption of the scapular cortex at the glenoid on the PA view, suggesting a scaphoid fracture.
What would you recommend next? History: New tachycardia. CT scan from today and previous CT scan of the abdomen from 6 months before.
What are the findings on  the new scan? Multiple pulmonary emboli in the bilateral lobar branches. Mild enlargement of the pulmonary artery.
What findings have changed in the interim? New enlargement of the pulmonary artery.
What are the critical findings to tell the ordering clinician? Possible early right strain with interval enlargement of the pulmonary artery and multiple pulmonary emboli.
