History: Chest pain.
What are the most important findings? Increased enlargement of the distal thoracic aneurysm with an increased mural hematoma. Poor opacification of the distal thoracic and upper abdominal aorta. Question flap in the aorta
What are the possible diagnoses? an admixture of blood products and contrast in the distal thoracic aorta vs. early dissection flap. Accompanying ruptured plaque with a mural hematoma.
What should you do next? Either follow up with another CTA with improved contrast bolus or MRA of the chest/abdomen.

What are the most important findings? Stranding around the head of the pancreas, SMV thrombosis, new gastric sleeve/post bariatric surgery changes
What is the most likely diagnosis? SMV thrombosis with accompanying pancreatitis or associated inflammatory change
What are the most likely causes of the diagnosis? post bariatric surgery pancreatitis with acute onset of SMV thrombosis or post surgical SMV thrombosis.
