History: Bump on finger.
Describe the findings. Coarse soft tissue calcification at the volar soft tissues of the first digit.
What is the differential diagnosis? Dermatomyositis, Tophus from gout, Calcified hemorrhage, Metastatic calcification (from metabolic disease like hyperparathyroidism)
What is the most likely diagnosis? Typical appearance/location for calcification from dermatomyositis.

History: Shortness of breath.
What is the critical finding on the chest film? Possible free air underneath the right hemidiaphragm.
What would you recommend next if the patient is not that sick? PA and lateral chest films or upright obstructive series.
What would you recommend next if the patient is sick? CT scan of the abdomen and pelvis.
What is the final diagnosis based on the CT scan? Interposed bowel loop anterior to the liver edge. No free air. “Chilaiditi’s Syndrome”

History: Pelvic pain.
What are the findings/diagnosis? IUD at the lower uterine segment and protruding into the myometrium (incorrect positioning)
What is the next step for the ordering physician? Consider hysteroscopy for removal of the IUD. (Laparoscopy if no portion of the IUD is within the endometrial cavity)

How is this condition treated? Cecopexy if bowel is not ischemic/damaged. Bowel resection if ischemic.
