Case of the Week From 7/25/21

History: Arthritis. Hip pain.

What are the findings? Edema within the gluteus medius muscle.
What question should you ask the patient to confirm the diagnosis? You should ask if the patient has any trauma history.
What is the most likely diagnosis? Gluteus medius muscle strain. (nerve injury is less likely but within the differential diagnosis)

 

Case of the Week From 7/18/21

History: Altered mental status. Request for CTA.

What is the key urgent finding/diagnosis based on the CT findings? Anterior aortic arch pseudoaneurysm. Key point of the case (always cheek all the images to the level of the aortic arch!)
Who should you call and why? Call vascular surgery directly because of risk of rupture.

 

Case of the Week From 7/11/21

History: Abdominal pain.

What is the most likely cause for the symptoms based on the CT findings? Acute diverticulitis of the descending colon with possible extraluminal air adjacent to the diverticulum.
What are the key pieces of information that you need to tell the emergency room physician and why? You need to say that the patient may have diverticulitis with a contained perforation (extraluminal air that is not free air) because the emergency department will keep the patient overnight with antibiotics to monitor for worsening symptoms.

 

Case of the Week From 7/4/21

History: Post prandial pain.

What is a possible diagnosis based on the CT findings? Median arcuate ligament syndrome (narrowing of the proximal celiac artery with post narrowing dilatation best seen on the sagittal view)
How specific is this diagnosis? Not a specific finding. Can be entertained depending on the patient’s symptoms/clinical picture.
