What are the key findings, the most likely diagnosis, and the next step in management? Dudodenal wall thickening, most likely related to dudoenitis, test for H. Pylori

 

 

 

This Week’s Case of the Week From 7/20/19:
History: Shortness of breath.
What are the key findings and critical diagnoses? Right hilar/precarinal/paratracheal mass with occlusion of the SVC and slight narrowing of the right pulmonary artery. Consider lung cancer. Collateralization of mediastinal and internal mammary vessels with additional recanalization and enhancement at the falciform ligament. Accompanying right-sided pleural effusion with adjacent atelectasis or pneumonia.

History: Change in mental status
What are the main findings and most likely diagnosis? Interval development of diffuse cerebral edema with loss of gray-white matter differentiation and sulcal effacement, Anoxic injury
How do you manage the patient? Call Neurology

History: Interval development of new back pain.
What are the main findings and most likely diagnosis? New erosive changes at the endplates of L4-L5. Consider most likely discitis/osteomyelitis. Needs contrast-enhanced MRI. (This case was path proven!)
